A família, a morte e o luto: o caso dos familiares que acompanha o doente oncológico em fase terminal no HOS

Abstract

O cancro é a segunda causa de morte em Portugal. A doença oncológica surge na vida do indivíduo quase sempre como doença prolongada, com grande sofrimento para o doente e família. A permanência do doente no seu domicílio proporciona mais oportunidade para trocas afectivas e emocionais, maior conforto e familiaridade do ambiente, maior autonomia na escolha de actividades. A família, na maioria das situações, não está preparada para acompanhar o doente no domicílio, principalmente, se este se encontrar em fase terminal. Acompanhar o doente em fase terminal, desencadeia no familiar emoções que se prendem com o facto de se ver confrontado com a proximidade da morte e por outro lado com o sofrimento do doente, ao qual muitas vezes não consegue dar resposta. Face à proximidade da morte o familiar sente medo da enfrentar e tenta adiá-la, pelo que recorre ao internamento do doente, lançando-o para uma morte solitária e contrária à sua vontade. A morte do doente reflecte-se naqueles que ficam. Os rituais fúnebres, o mundo do trabalho, as tarefas quotidianas que os mantém em contacto com a sociedade e com a comunidade, os amigos, os vizinhos, poderão ser um suporte no processo de luto. Sendo a equidade social um factor preponderante para o desenvolvimento, será também ao nível familiar que se poderão encontrar respostas para a não exclusão dos doentes em fim de vida. Este estudo de caso, analisa a realidade do familiar que acompanha o doente oncológico em fim de vida no domicílio, tendo como objectivo geral - Analisar o processo de luto dos familiares face à morte do doente oncológico terminal no domicílio. /***Abstract - Cancer is the second cause of death in Portugal. Oncology diseases appear in one's life usually in the form of a prolonged disease with a great deal of suffering for both the patient and his family. Being at home allows the patient to have more affectionate and emotional contacts, to feel more comfortable and relaxed in his own surroundings and to have a greater autonomy to choose his activities. Family, most of the times, are not prepared to attend the patient at home especially if he is terminally ill. To attend a terminally ill patient stirs in his family emotions connected not only with their feeling of proximity to death but also with the suffering of the patient, which they cannot alleviate. When death is near, the relative fears to face it and tries to postpone the inevitable by taking the patient into a hospital for treatment and thus throwing him into a lonely death against his will. The patient's death affects deeply those who stay behind. The funeral rites, work, the daily routines that keep them in contact with society, with their community, with their friends and neighbours may help them bear with mourning. Social equity is one of the main factors of development, and maybe in a family level you will find solutions for not excluding terminally ill patients. This study analyses the situation of a relative who attends at home a patient with terminal cancer and its general purpose is - To analyse the process of mourning the death at home of a terminally ill cancer patient by his relatives.
